My question for you guys is this: What impact does ingesting these spices have while on a ketogenic diet specifically on BHOB blood ketone levels. I currently take fenugreek because I learned about the glucose lowering effects in a study when it slowed the absorption of the carbage these SAD dieters were eating. But since a well-formulated keto diet has very few carbs to begin with, would the fenugreek provide the same benefit or may it even backfire given that fenugreek is almost 60% carbohydrates? I realize blood sugar lowering isn’t the same as ketone-boosting, but I do see the two going hand-in-hand. So what do you think? Does a ketogenic diet lower insulin levels and provide ultimate healing for insulin resistance which seems to be genetic? Hi guys, I'm a recovered diabetic thanks to eating a low carb (but not necessarily ketogenic) diet. I used to be on the maximum daily Metformin dosage and took 130 units a day of long-acting insulin. I’m off all of those medications now and have an A1c of 5.3. I lost 60 pounds fairly easily on low-carb, but now it’s stalled out. My insulin levels are still very high, so I assume I’m still quite insulin resistant which is making the weight loss stubborn. I’ve added in some intermittent fasting of 18/6 as well as a few 24-hour fasts on the weekend where my blood ketone levels have reached about 1.0 mmol. My question for you guys is this: Does your experience show that a truly ketogenic diet lowers insulin levels? Some experts I’ve heard have said that research shows that insulin resistance is pretty much genetic which means you may not be able to drop it below some arbitrary level no matter what you do. I have used a CPAP machine for 15+ years and I wear it faithfully every night. I started keto in January 2016 and have noticed since eating this way that I don't have as many aches and pains as I once did. I have only been able to lose 10 or so pounds, my blood pressure is somewhat high at 138/85, my AC1 is 5.7, and my urine cortisol levels are fairly high at 188. Could incorrect pressure in my CPAP machine have anything to do with not being able to lose weight despite the fact that I’m burning ketones according to the Ketonix which always shows me blowing red for higher levels of ketosis? Thank you for your time, Kurt 3. Can a high-dose Vitamin C IV treatment spike blood sugar levels alone or is there hidden glucose causing this? Hey Guys, We love your podcast and anxiously await the new episodes each week. My wife and I have been low-carb for two years and keto most of the time for the past year. I travel about 18 days a month so it’s hard to maintain a state of nutritional ketosis all the time. We are 60 and 61 years old and we keep our blood ketone levels between 0.5 to 1.0 most of the time. Our question is about rising blood sugar levels after getting a Vitamin C IV from our functional medical doctor. My wife and I have both been getting this treatment lately because our bloodwork showed that we are low on Vitamin B and C even though we take supplements for both of them. After the first Vitamin C IV treatment, my blood glucose level rose to 172 and my wife’s went to 284 about 1 hour after the treatment. Our normal fasting blood glucose levels are between 77-95 depending on how we ate the day before. Needless to say, we became concerned this would have a negative impact on our insulin resistance. When we asked our doctor about it thinking that there must be some glucose in the IV bag, she said that it was just a reaction to the high dose vitamin C and that there was no glucose in it. We had already paid for two treatments each, so we did it again this week and this time our blood sugar went up to 144 and 128 respectively an hour later. Have you ever heard of this kind of reaction or do you think that there was some glucose in the IV? Our doctor knows that we practice low-carb and she is monitoring us because she’s still quite skeptical. - What can I do to heal my adrenals and thyroid issues while on a ketogenic diet? Do I need to heal these prior to going keto?

Hey guys,

I have been attempting to do a ketogenic diet since last July and it works wonders for my MS symptoms (I first heard about it from Dr. Terry Wahls). The issue I'm having now is adrenal fatigue and thyroid issues. I’m taking low-dose Synthroid, but I’m tired, nauseous, depressed, and feel cold all the time. Despite keeping my carbs under 30g total daily eating only whole foods, I have gained 20 pounds in two months. Since the adrenals and thyroid are connected, is it possible that I’m eating too low-carb for my stressed out adrenals? Or I am I not low-carb enough? You guys often say on Keto Talk that hormones dictate everything about your weight and health, but something is going on with me that I can’t seem to figure out. I feel like crap most of the time despite being on the healthiest diet I've ever eaten in my life—no gluten, no soy, no dairy, no sugar, 3-5 cups of fresh veggies, full fat coconut milk, grass-fed meats, cooking with bacon grease, and 1/2 cup of blueberries daily. Is there a way to heal my adrenals and thyroid with the ketogenic diet or do I need to heal these first before going full keto? Really love the podcast.

Hey there,

I recently read this article by a naturopath named Dr. Lara Briden: http://www.larabriden.com/are-you-eating-enough-get-period-amenorrhea/

In the post, she claims that eating a low-carb, ketogenic diet will take away your period. I've still not had a period since I started keto over 20 months ago. I also developed all four of the signs she mentioned: low T3 (non-Hashimoto's hypothyroid), hair loss, constipation, and insomnia. I average 5.5 hours of less-than-restful sleep per night with sleep aids and high-dose melatonin waking up every couple of hours. I'm desperate to know how I can tweak my ketogenic diet to favor fertility. My naturopath knows very little about keto and I’m not sure she can help me. I’ve been strict keto with 20/4 intermittent fasting for a while, so I wonder if I could get away with some starch every few days to see if that helps. Between my infertility and profoundly low estradiol, it just know it has triggered my binge eating.

2. Should I eat both low-carb and low-calorie in order to get into ketosis and see the results I’m looking for?

Hi guys,

I have listened to all of your podcasts, some of them twice as I’ve become a sponge with the information. My question for you is about breast cancer and ketosis. I was diagnosed with breast cancer 3 1/2 years ago and have had radiation and on tamoxifen for over three years. Cancer is what led me to start listening to health podcasts. During the cancer treatments, I put on 5kg (11 pounds) and it all seemed to develop in my midsection. I have tried low-carb and even keto, but can’t seem to get into ketosis no matter what I do. You guys have said many times on your podcast that some women need estrogen to help with weight loss, but I’m wondering if perhaps I need to be eating low-carb AND low-calorie in order to get the best results. When I’ve done keto previous about 8 years ago when I was at a bodybuilding gym, I lost a significant amount of weight very quickly. But I just can’t seem to get there now. I also take antidepressants to help with hot flashes and realize these have a side effect of weight gain. Can you help me?

3. How can I get the benefits that ketosis provides my autoimmune arthritis without the unwanted weight loss that happens when I eat keto?

I have been enjoying your podcasts and reading your Keto Clarity book. I started on a ketogenic diet to help with my autoimmune arthritis and I think I've been making some progress. Unfortunately, I’m losing weight and I don’t need to be. I’m 5’2” tall and actually underweight at 91 pounds. My goal is to be at 105 pounds. Despite shoveling down 75% fat in my diet, I just keep losing weight. Tracking my nutrition with an online tracker, it says my calories are higher than are required for me and yet my weight keeps dropping despite the fact I feel good. Should I choose being in ketosis and controlling my arthritis while losing weight or eating a few more carbohydrates to get out of ketosis in order to get back to normal weight again but dealing with the arthritis?

Got any suggestions? I'd so love to get back some of my feminine curves again.

- How do I need to adjust the macronutrients on my ketogenic diet in the late stages of my pregnancy and while breastfeeding? Should I add more protein?

Hi guys,

Love the show! You are doing an amazing service, so keep up the good work. I’ve been keto for four months and I’m doing great losing 25 pounds and dropping my A1c from 5.6 to 5.0 in just two months of eating this way. My husband went keto at the same time as me and has lost 40 pounds. Needless to say, our doctor is ecstatic about our results. We’ve heard people joking about being careful when you go keto and normalize your hormones because it might just make getting pregnant easier—well SURPRISE, here comes baby #4!

Are there any recommendations about how to adjust the macronutrients of my ketogenic diet for the second and third trimester and even once I started breastfeeding? I usually eat around 80% fat in my diet with protein around 70g and carbs at 20g. I’m definitely making sure I consume at minimum of 2,000 calories daily so I don’t stress my Hashimoto’s or my adrenals that I’ve worked so hard to heal over the past four years. Should I eat a bit more protein as the pregnancy continues? You guys will find this interesting that the way I found out I was pregnant was my ketones suddenly dropped to 0.3 after always registering 1.5-3.0 and on a hunch we did a pregnancy test. Whammo bammo, pregnant. Crazy right?

I’m so thankful to be keto-adapted heading into this pregnancy and hoping it will keep my notorious severe morning sickness at bay this time around.

I understand that blood glucose can rise on a ketogenic diet due to the physiologic glucose sparing effect you guys have discussed several times on the podcast. In his new book FAT FOR FUEL, Dr. Joe Mercola shares his solution to this by simply eating more carbs and thus trigger an insulin response to bring the glucose down. Of course, if you eat more carbohydrates in your diet to make this happen, it will invariably kick you out of ketosis. So in light of this, it begs the question: which is worse for you to have—higher glucose levels or higher insulin levels? Is there another way to bring blood glucose down without triggering insulin to go up? Maybe some light exercise would do the trick? Thanks for answering my question.

1. In light of my blood work results, is it possible keto isn’t right for me? If I want longevity, energy, and mental benefits, how should I eat?

Hi Jimmy and Adam,

Thank you for all that you're putting out there regarding ketogenic living. I first came across you Jimmy when you did a podcast with Dave Asprey a few years back and have listened to all of your Livin’ La Vida Low Carb Show and Keto Talk episodes ever since.

I've been experimenting with keto on and off for the past few years. I don't do it for weight loss since I'm a lean 32 year old male at 8% body fat with high muscle mass and in very good shape (I can run a sub-6 minute mile, deadlift 3x my body weight, etc.). I follow keto for constant energy, mental acuity and longevity purposes.

When measuring my blood glucose levels while in keto, I find that they're quite high: 100-120 fasting blood glucose and up to 140 postprandial. I'm aware of physiologic insulin resistance, but the numbers nonetheless concern me; I just had blood labs done and my HbA1c was 5.8, up from 5.2 which were my numbers a year ago while on a moderate carb diet.

My triglycerides have also gone up from 54 in February on moderate carb to 97 today. My total cholesterol has risen from 180 to 225 but that's to be expected on keto. My testosterone is a bit on the low side for my age at 450, but it’s been this way regardless of whether I'm on keto, moderate carb, or have taken a week off of training (I was hoping keto would raise it).

Dr. Dominic D'Agostino acknowledges that for certain people, keto simply isn't the answer. Although I feel healthy eating this way, my blood work results seem to indicate otherwise. Am I one of those people who flourishes on moderate carb eating and the occasional fast and keto just isn't for someone like me? It's worth noting that my mother who is in her mid-60's is in spectacular shape with great lab numbers without ever going to the gym and she has always eaten a carb-laden diet.

One more interesting note to share with you is my body composition is identical whether I’m eating keto or high carb. So, with longevity being one of my top priorities alongside stable energy and mental acuity, what nutritional approach do you think I should do to reach my goals?

2. Is it possible to create acidosis from your diet and exercise choices? How can you prevent this from happening?

Hi guys,

Thank you for your work as it's an absolute asset to so many of us. I started doing 2 workouts a day of weight lifting and running or walking. I’ve noticed I started having painful urination as well as my side started hurting recently. When I checked my urine ketone levels several times, they were always extremely dark. After reading a graph on Dr. Andreas Eenfeldt’s website about acidosis, I became concerned. I immediately ate a bunch of carbs from ice cream and other sources to try to deal with this issue and stop the pain by driving down the ketones. I also drank some baking soda mixed with water since I thought this had to do with acidity. Amazingly, 8 hours after doing this I STILL showed ketones on the urine ketone sticks. So then I ate some chili cheese fries bringing my total carb consumption that day to 500g.

It's now been 3 days later and my body had never stopped producing ketones. It went to small levels at one point when I skipped my workout for a day. I'm a type 2 diabetic with chronic kidney disease who's used straight keto for about a year and have never seen anything like this. If I have a carb-filled meal, this will usually kick me out of ketosis for 4-5 days. So, have you seen acidosis happen as a result of diet and exercise? What are some things I can do to prevent that from happening again?

3. Is there any added benefit to taking one month off from keto annually vs. constantly staying in nutritional ketosis?

Hey Jimmy and The Doc,

I have listened to most of your Keto Talk podcasts as well as some LLVLC podcasts and they are both informative and entertaining. What I take from hearing people like Dr. Eric Westman, Dr. Ted Naiman and Dr. Nally is that the hormone insulin is what controls and signals the storage or burning of fat and that lower insulin levels (outside of being a Type 1 diabetic) is healthier. It seems that intermittent fasting, keto, and exercise all lower insulin and improve health. Many of the arguments point to the evolution and hunter gatherer hypothesis of storing fat in times of plenty during the spring and summer and then burning stored body fat during the winter and when food was in shortage between animal kills. The problem with the modern-day SAD diet is that there are too many carbs in our diet that leads to elevated insulin levels which then results in insulin resistance and your body is constantly storing fat, never using it in times of need which never come, and leaves the individual perpetually hungry to eat and eat and eat.

My question for you guys is this: if we are designed to store and burn fat and alternate from using ketones and glucose as our fuel source, then isn’t this an argument for cycling in and out of ketosis on a regular basis? Since getting keto-adapted takes about 4-6 weeks for most people, why wouldn’t doing 11 months of keto with one month of a non-ketogenic diet be better than eating keto 24/7/365? I have not found any studies on this and I would imagine the change in diet would be good for the body to serve as reset for just one month a year. I know yo-yo dieting is bad for your overall health and that there is a risk of a relapse to your old ways when you get off of keto that could prevent you from going back to it again. But there might be real benefits to this as it probably better resembles how our ancestors evolved in their nutrition and may help reset your body to get better results after long-term ketosis. What do you think?

I have a quick question you guys: I recently got my Pathway Genomics results and the report said that my genes would benefit from a LOW FAT, higher carb diet. I have been keto for a year and I was shocked to see this. The report suggested my fats should be comprised of 20-25% of my diet, protein 20-25% and carbs 50-55%! I shuddered when I read this. It also said my body can process monounsaturated fats better than animal fats. What gives? Do my genes just not lend themselves to a keto diet?

1. Why did my symptoms of Raynaud’s syndrome seem to get worse when I started eating keto?

Hey Jimmy and The Doc, I went gluten free years ago and Paleo was the next natural step. I have had autoimmune issues my whole life and my doctors couldn’t figure it out. One of them once told me, "we usually only see these symptoms in overweight people", but overweight I am not. When first going Paleo, I made a drastic mistake without knowing it. The Paleo crowd was all about the bananas and potatoes back then so that's what I did. Even though I am not big fans of either of these they were the least offensive carbs and it seemed to help. A few months ago I decided to go with a ketogenic approach doing it the correct way this time. I still have fruit in the house for my kids but I have not touched it. I never had the low-carb flu and within a week was sleeping like a rock for the first time in years despite living the stressful life of being a single mom. Joint pain did not flare up as much and my migraines are not showing up as often. My question for you guys is this: I have always been plagued with Raynaud's syndrome. It lasts all year long, not just in the winter. I have noticed that since I do not feel hungry on a regular basis it is easy to go many hours without food while out running errands without noticing how long it has been. My Raynaud’s then rears it's ugly head and I feel it. I'm not sure if it's going too long on an empty stomach or something else happening, but the Raynaud's is the only thing that has gotten worse since I started eating keto. Do you know why this is happening? I am already taking a magnesium supplement to help. Even if you guys have no solution for me, I will just suck it up buttercup and deal with it as I have in the past because the benefits I am getting from nutritional ketosis far outweigh the cold hands and feet that I have always had to deal with anyway. Listening to Keto Talk, it is very clear to me that you guys have a goal of helping people and getting correct information out there. I am so glad I found your podcast.

2. Why did I start experiencing very sore spots on my skin when I decided to cheat on my low-carb, ketogenic diet? Hey Jimmy and The Doc, I recently found your podcast and have greatly enjoyed getting caught up on all the episodes! I have a question that might actually be a new one for you as I haven’t heard it discussed on your show before. I have been eating low-carb, ketogenic with weekly cheat days as Tim Ferriss outlines in his book The Four-Hour Body. After about six months of doing this, I noticed I would wake up with very sore spots on my skin following a cheat day. It seemed to be concentrated on my torso and sometimes on my upper arms, neck, and scalp. This soreness was on the surface of the skin, not the muscles or joints. The best way I can describe it is it felt like a giant bruise that was only painful when you pressed on it. My skin would be so sensitive that I would skip running or other exercise that day because the mere bouncing of my skin was too uncomfortable to bear. The surface area of skin that would be sore seemed to correlate positively with how much crappy carbage I splurged on the previous day. Interestingly, the first areas to become sore were the areas in which I tend to store fat, such as the love handles, and the soreness would radiate out from there. If I had a really obscene cheat day, then even my upper leg skin would get sore. Very surprisingly, this phenomenon did not go away when I went back to the high-carb Standard American Diet for a period of time. It seemed anytime I indulged in something particularly sugary/carby, my skin got sore. This soreness would only subside after a day of eating low-carb keto again. So my questions for you guys is this: What could be causing this? Is it just a rapid-onset generalized edema resulting from a significant increase in carbs? Why did it not begin occurring until a good 6+ months into eating low-carb and why did it persist after I went back on a high-carb diet? Have I permanently altered how my body reacts to carbs by going low-carb for so long? Finally, should I be concerned about this? While trying to find answers to these questions online, I saw this “sore skin” phenomenon mentioned by a couple of people on low-carb diet forums who claimed they never experienced this before going low-carb. But none of these references included any explanations other than "possible edema".

3. Can Adderall be having an adverse effect on getting into ketosis? And does keto do anything to help improve adult ADHD? Dear Keto Talk hosts, I am a 62-year old female and have been moving to keto through the usual progression—it started 20 years ago trying to eat local and real food, then on to WAPF, then Paleo, and now low-carb, ketogenic consuming 20-40g carbohydrates daily. Most of my carbs come from salad, kefir, cheese, and kombucha. My blood sugars have been coming down but I am still in the high 90s for fasting blood sugar in the morning. I have only seen .5 once on the blood ketone meter. I am trying to figure out what might be preventing ketosis for me and I have a couple of suspects. Because I have some of the MTHFR family of variations, my doctor has me on 1000 mcg of L-methylfolate and adenosyl/hydroxy B12. I am also taking 10 mg daily of Adderall. Would any of these be having an adverse effect on getting into ketosis? And does ketosis have any positive effect on improving adult ADHD? I’d really love to get off of that medication if at all possible. Thanks for any insights you can provide. Stephanie KETO TALK MAILBOX - Why can’t I stay in ketosis eating what seems to be a well-formulated low-carb, moderate protein, high-fat, ketogenic diet? Hey Jimmy and The Doc, I listen to your Keto Talk podcast all the time and I have been doing keto for nearly 2 months. I am 6'7" and weigh 220 pounds with 16% body fat. I started keto for all the health benefits, energy, and disease prevention it provides. I eat about 100g protein, less than 20g carbs, and 200g of fat. This all seems right to me and I am being very strict with it. I get lots of sleep, am not stressed, and have experimented with exercising a little and not doing any exercise. There’s only one problem: I JUST CANT STAY IN KETOSIS! I bought a blood ketone monitor and the highest reading I have seen is 1.2 for a very short 2-day period and most of the time I am less than 0.5. Every morning I wake up I am 0.2 and then it’ll rise to 0.6 mid day and once in awhile I'll get to 0.8 in the evening or sometimes it goes the other way and I'll drop back down to 0.3. I eat bacon and eggs in the morning with coffee, MCT oil, and full fat whipping cream as well as high-fat meats and cheeses or cream cheeses for lunch and supper. The frustrating thing for me is my wife is getting 2.5-3.5 on the ketone meter eating the exact same thing I am. HELP! I hope you have an idea why I can't stay in ketosis.

- Is there concern about very high levels of bile acid from a high-fat ketogenic diet leading to an increased cancer risk?

Hi Jimmy and Adam,

I was listening to Dr. Rhonda Patrick on Joe Rogan’s podcast where she mentioned that bile acid, namely, deoxycholic acid, is necessary to emulsify fat for absorption in the intestine and that a high consumption of fat – over 60% of calories - on a regular basis means the body is subject to higher levels of this acid. She stated outright that deoxycholic acid in higher than usual quantities causes damage to the DNA, even going so far as describing it as carcinogenic. Dr. Patrick noted that a ketogenic diet will ultimately result in the failure of the enzymes whose function is to repair DNA and thus will put the person at greater risk of cancer.

I am a Type 2 diabetic and have maintained a ketogenic diet for the past year. I was persuaded to adopt this diet by reading Keto Clarity and am regularly in a state of nutritional ketosis. My last HbA1c was 5.2% and my fasting insulin level was 4. I can’t imagine adopting some other approach despite the concerns about the higher levels of deoxycholic acid required to emulsify the fat. But is Dr. Patrick correct in her assessment of this cancer risk with keto? Thanks for your help!

1. How do you handle dealing with hypoglycemic episode when you go on a ketogenic diet? Does adding in carbs resolve the issue?

Thanks for a great podcast! I really enjoy the information, and particularly when The Doc geeks out on us. My question for you guys is about hypoglycemia. If a person starting keto experiences a hypoglycemic episode, what’s the best way to deal with it? I can do 2-3 days of a keto diet but then on the 3rd or 4th morning I wake up with low blood sugar levels and experience nausea, feeling shaky, unfocused, and sometimes seeing brown spots. My blood glucose dips into the 40s and resolves after eating an apple, or some raisins, or some sweet potato. But then I feel like crap for the whole day. I don’t get this reaction when I simply eat lower carb with some sweet potatoes or fruit during the day. But I really want to experience all the full benefits of being ketogenic. I know I can't possibly be the only one who struggles with this.

3. Does consuming fermented dairy like cultured butter cause a rise in insulin to cause hunger? And will eating this kick me out of ketosis?

I love your show! Although I’ve been at a healthy weight all my life, I’ve decided to follow a ketogenic diet to have a more positive relationship with food and to balance my moods. Your podcast has greatly inspired me! I recently bought a pack of cultured butter at the grocery store and after having some for the first time, I noticed that I was craving more of it which usually never happens when I consume fat. After doing some research I discovered that fermented dairy products tend to have a higher glycemic index than non-fermented. What is the impact of cultured butter on the hormones that control hunger and cravings? And could consuming fermented dairy products knock me out of ketosis?

- Is it common knowledge that the AMA promotes ketosis for a greater sense of well-being and pain reduction at the end of life?

Hey Jimmy and Adam,

I just lost my grandfather to pancreatic cancer. He chose to have no artificial nutrition through a feeding tube and IV in the last few days of his life. I believe that this allowed him to go into ketosis and eased his passing. I also found a reference to ketosis being beneficial at the end of life in an American Medical Association (AMA) publication. In researching this further, I found the following statement in the Education for Physicians on End-of-life Care Handbook by the AMA:

"Educate families about the studies that demonstrate that parenteral or enteral feeding of patients at the end of their lives neither improves symptom control nor lengthens life. Help them to understand that anorexia may be protective, as the resulting ketosis can lead to a greater sense of well-being and diminish pain."

So my questions for you guys is this: Is this common knowledge in the medical community in general? We were never told this by any of my grandfather’s medical caretakers and instead were encouraged to have him drink Boost with 20-30g sugar/serving right to the end. In light of this, could exogenous ketones have helped him ease the pain more and remain lucid enough to engage with the family a period of time longer before passing?

I really hope that you can share about this on Keto Talk. At least this community may be able to help a dying loved one by NOT pushing sugary drinks on them even when the nurses and doctors insist. Or possibly asking for no dextrose in their IV, especially if the patient has already said that he is ready to pass. It may be helpful to share the AMA reference with family members and caregivers, and explain what ketosis is and that the patient may not be feeling hunger. Those of us who have experienced ketosis know this firsthand.

It takes courage to go against mainstream thinking, especially in the world of medicine and health. I appreciate that you do it every day with your podcasts, blogs, and books.
